Making Arrangements at a Nursing Home


Your decision is made and you have selected a nursing home. You’ll need to contact the nursing home to make admission arrangements. Contact information can be found on the nursing home’s page on WhereToFindCare.com. Some nursing homes have contact forms on their page so contact can be as easily as submitting the form.

The nursing home admission director will walk you through the whole process. Below is a list of information commonly asked for in the admission process*:
  • Payment information: Gather insurance information and the policy numbers
  • Medical History
  • Current Health Status
  • List of current medicines
  • List of current health care providers (include names and contact information)
  • List of emergency contacts (include names and contact information)
  • Health Care Advanced Directives. These are written documents that state how you want your medical decisions made in the case you are unable to make them yourself. If you don’t have advanced directives or need help, contact your current doctor or the nursing home staff.
